WHAT IS THE COMMUNITY CONNECTOR PROGRAM?
The Community Connector Program is a community-based approach using trained community members to identify at-risk older adults.
WHO ARE COMMUNITY CONNECTORS?
Ideal Community Connectors are individuals who come into contact with older adults through their work or their daily activities.
HOW CAN I LEARN TO BE A COMMUNITY CONNECTOR?
You can sign-up for a workshop that educates Community Connectors about the impacts surrounding social isolation and loneliness in older adults and how to refer isolated older adults to community supports.
